We believe that comprehensive health care is a human right. That’s why our services and programs are open to all who need us, regardless of the ability to pay, without judgment or exception.
Wellness Bar accepts most health insurance plans including HMO/PPOs, Medicaid, Medicare and CHIP. We also offer programs to lower the cost of health care services. Insurance and eligibility assistance is available at all Legacy locations. You may make an appointment with one of our Legacy Eligibility specialists by calling (832) 548 5000 or asking a front desk staff member. If You Are Currently Uninsured, Legacy Can Help You Access Health Care
Accessing health care can be frustrating, confusing, and even intimidating. If you are currently uninsured, Legacy’s Eligibility specialists can help you navigate the health care landscape and help you find the right combination of grants, government assistance, and sliding fee scale to make your health care more affordable. Our Wellness Hosts can help you schedule an appointment with a Legacy Eligibility specialist at your convenience.
Sliding Fee Scale
Patients who do not qualify for assistance programs, can receive a medical visit for a lower rate based on income and family size. Sliding Fee Scale is determined at your initial visit to Wellness Bar.

You may make an appointment with an Eligibility specialist by asking a Wellness Host or by calling (832) 548 5223. You will need to supply proof of your identity, income, and other information.
While our goal is to provide affordable care for all our patients, payment must be made at the time of service. Not having payment at the time of service may result in your appointment being rescheduled.
Health Insurance Plans Accepted
Wellness Bar by Legacy currently accepts the health insurance plans listed below. In order to better serve you, changes are made to this list over time. Please address any questions about your plan with a Legacy team member when making your appointment.
Please remember to show us your insurance card when you arrive for your appointment. If you do not let us know about your coverage, you will be charged for past visits. For patients with a plan that needs a primary care provider (PCP) to be listed, either Legacy or a Legacy provider must be your PCP with your plan.
